Please note that in order to book an appointment, you will be asked to read and sign the following statement as part of your initial paperwork.
Data Protection:
Please be aware that by sending back the completed form you are consenting to your data being securely stored and processed by myself, (Jo Southall, Independent Occupational Therapist). This data is only stored for the purposes of providing Occupational Therapy services and to facilitate communication between you (service user) and me (therapist). I will also keep notes about our interactions and summaries of each session undertaken, this creates a narrative of your individual therapy experience. You can be provided with copies of this within 1 month of a written or verbal request.
Right to withdraw:
You or I can postpone or terminate services at any time if continued intervention is no longer in the best interests of you (the service user) or me (therapist). You have the right to withdraw consent for therapy and/or data storage at any time, in this case all personal data will be destroyed or returned to you and all future therapy will be cancelled.
Video Chat Statement:
Due to my personal circumstances, appointments are available almost exclusively via video chat. If you decide to engage in therapy using these means it’s important to be aware that no online communication is 100% secure. Some forms of online communication are considerably more secure than others. I have taken every precaution to ensure that email communications are secure, encrypted and password protected. All information stored regarding ongoing or past therapy will be backed up to the cloud behind a two-step authentication process on a password protected work laptop. Information passed via third party service providers such as Facebook, Skype, Zoom and Google cannot be guaranteed secure. In light of this, please only pass identifying, personal or confidential information via email to hello@jboccupationaltherapy.co.uk and not by social media or instant message.
Occasionally, technical difficulties can make for poor quality video, sound or connection. If this cannot reasonably be solved during the appointment a new appointment will be arranged.
Cancellation Policy:
Due to the complex and unpredictable nature of many long-term conditions, there is no deadline on cancelling or re-booking appointments if needed. A non-refundable deposit will be taken to confirm your initial booking. This entitles you to re-book as many times as you need to. In order to make the most of your sessions I advise a decent level of cognitive function. In line with this, I actively encourage you to self-manage your symptoms in any way you need to. I have no problems delivering a session with you wearing PJ’s or laying on the sofa. Occasionally, I might wear PJ’s or lay on the sofa too.
